Online Toolkit for Physicians

The Illinois Academy of Family Physicians has created a comprehensive list of best practice, evidence-based resources to support improvements in the early detection, diagnosis, and treatment of Alzheimer’s disease and related dementias. Our goal is to support Illinois Primary Care physicians and their healthcare teams as they work to enhance clinical workflows to help patients receive support sooner. The IAFP will maintain and update this toolkit as new resources are made available.
